Registration Process - Applications sent by mail
1. Can I send the form first, then forward the cheque at a later date?
The form and the cheque must be sent together for the registration process to begin.
2. Do I send or fax the form to my local Alzheimer Society or the national office?
You can mail the registration form and cheque directly to the Alzheimer Society of Canada (national office). If you have been in contact with your local Alzheimer Society, or want to be, you can also send or drop off the completed form and cheque, and they will forward this information to the national office.
Please note: Photocopied or faxed forms will not be accepted.
If you haven't already been in touch with your local Alzheimer Society, you might want to contact them to find out what other services and programs are available in your community in addition to Safely Home.
3. Once I register, what do I receive in the registration package?
The registration package will contain an identification bracelet, a Caregiver Handbook and three identification cards that can be kept in the Member's wallet and/or purse.
4. How long will it take to receive the registration package?
It takes approximately four weeks to receive your registration package, once the Alzheimer Society of Canada (national office) has received your completed form and cheque.
5. If I need to change any information, how do I do that?
We accept updated information by mail, fax or e-mail. You do not need to complete a second registration form. A letter or e-mail written by the contact person is sufficient. To ensure accuracy, we do not accept updated information by phone.
You may also update your member's record online, upon signing into our website. If you do not have sign in information, please contact the Alzheimer Society of Canada for a username and password to access your record.
6. If the Member moves to a care facility, should she still remain registered with Safely Home?
It is a very good idea to keep the person registered, especially if she is able to walk and has had a history of wandering. Caregivers or family members should notify the Safely Home Coordinator of any changes including the name, address and phone number of the care facility, and should advise the administrator of the care facility that the person is registered with Safely Home. The member should wear their Safely Home bracelet at all times. Please note that people in wheelchairs are still at risk of becoming lost from a care facility.
Bracelets
1. What is the standard bracelet size for men? For women?
The standard bracelet size for a man is 7.5" and the average size for a woman is 7". Please indicate the appropriate size of the bracelet on the form (including an added half inch for comfort).The bracelet only comes in one standard style, a stainless steel wrist bracelet. Any other styles of bracelets can be obtained through your local jeweler.
2. What is the fee to replace a lost bracelet?
The fee to replace a lost bracelet is $15. You can mail the cheque directly to the Alzheimer Society of Canada. When mailing the cheque, please include the member's last name, first name and, if possible, the registration number on a separate piece of paper.

4. What if the Member is allergic to metal?
The bracelet is made of stainless steel with the following composition: chrome 18%; nickel 8%; low carbon steel 74%. If a metal allergy is a concern, you may coat the bracelet with clear nail polish. If you would like to have a jeweler produce a bracelet of another metal (silver, gold, etc), you may do so but you need to include all the information that is on the Safely Home bracelet.
5. What if the person takes off the bracelet and won't wear it?
One strategy you might try is to present the bracelet as a gift. This may help the person accept it more easily. Check that the bracelet is not too tight or too loose. Is it uncomfortable in any way? Consider purchasing a bracelet style at your local jewelry store that is similar to what the person is used to wearing (e.g. a watchband or bangle).
You'll also find three identification cards in the back sleeve of the Caregiver Handbook that you'll receive. Be sure to fill these out and put one in the member's purse or wallet.
